Our method for carrying out this mission are primarily threefold.

1.  Work with individuals to develop plans for their care which focus on the whole person (body, mind, and spirit), promote self-care, and provide for improved health.

We receive referrals from agencies, faith communities, and individuals in need of assistance in meeting their health care needs;

Those seeking assistance will be provided assistance by health professionals and volunteers with special talents.

2. Work with key referral sources to identify and serve people needing individualized assistance accessing resources and coordinating their care.

We consult with health and social service professionals about people in our community who have difficulty accessing resources and coordinating their health care and the lack of assistance/gaps in services available to these people;

We also collaborate with health and social professionals to provide specific referral and care coordination services that are not currently being offered to people experiencing health challenges;

Additionally, we serve as a referral resource for health and social service professionals for people who need assistance in accessing resources and/or coordinating their health care.  

3. Work with faith/religious communities and individuals within these communities to identify and serve people needing individualized assistance accessing resources and coordinating their health care.  

Information is provided to local faith/religious communities of current needs in Waco of persons with health challenges and health ministry opportunities;

Information is offered to faith/religious communities about health ministry and community resources;

An invitation is extended to local faith/religious communities and individuals within these communities to partner with PVM to serve people with health challenges;

Referrals are made for people with health challenges to partnering faith/religious communities and individuals within these communities for assistance.
